For example:
❌ Problem: A quality inspection was missed.
Asking "Who forgot?" may identify the person.
Asking "Why was the inspection missed?" can reveal the process weakness.
Perhaps:
→ The task was not assigned
→ There was no reminder
→ Responsibility was unclear
→ The workflow lacked escalation
Fixing the root cause means fixing the system behind the problem.
